OLIVER SPRINGS YOUTH CLUB
UNIFORM & EQUIPMENT AGREEMENT

- I understand that Oliver Springs Youth Club will loan my child a uniform and/or equipment to use for
the season and that it is property of the Oliver Springs Youth Club and must be turned back in.
- I understand that I will be held liable for my child's uniform and/or equipment.
- I understand that I must turn my child's uniform and/or equipment back in when requested to at the end
of the season and get my child's name checked off of the outstanding uniform/equipment list.
- I understand that if my child quits before the season is over, that I am still responsible for turning in my
child's uniform and/or equipment and getting my child's name checked off of the outstanding
uniform/equipment list.
- I understand that when turning in my child's uniform and/or equipment that I must get his/her name
checked off of the outstanding uniform/equipment list at that time, otherwise I still must pay for that
uniform and/or equipment.
- I understand that the uniforms and equipment given out are not for sale and that the price paid for the
uniforms and equipment that are missing will be higher than what was originally paid for by the Oliver
Springs Youth Club.
- I understand that by not turning in my child's uniform and/or equipment AND by not making sure that
my child's name is checked off of the outstanding uniform/equipment list that my child will not be
allowed to play anymore sports through Oliver Springs Youth Club until the uniform and/or equipment
is either turned in, accounted for, or paid for.
- I understand that Oliver Springs Youth Club has every right to seek reimbursement for any uniforms
and/or equipment that is damaged, missing, or determined unusable due to negligence.
- I understand that this agreement is in effort to reduce uniform and equipment replacement
expenditures to the Oliver Springs Youth Club.
- I understand that the Oliver Springs Youth Club Board of Directors has voted on and approved this
agreement and that no exceptions will be made for anyone.
